We'll send paperwork through our secure patient portal to complete before your appointment.

Your First Visit
Arrive ready to share your history and goals. Initial visits are up to 60 minutes — no rushing.
What to Bring
Insurance card (if using insurance)
List of current medications and dosages
Any previous psychiatric or psychological records
School records or IEP (for children and adolescents)
List of prior diagnoses if applicable
A trusted family member or support person (optional but welcome)
What to Expect
Your initial visit is up to 60 minutes
You'll be asked about your history, symptoms, and goals
No pressure to commit to any treatment on the first visit
A follow-up plan will be discussed at the end of the visit

Please Note Before Scheduling
We are not able to provide:
- Neuropsychological testing or psychological testing
- Narcotics, opioids, or controlled pain medications
- FMLA or disability paperwork
- Forensic evaluations or custody case involvement
- After-hours emergency services
